CIU 2012 Membership DriveBlank Membership Application Form 2012 (Word format) Dear CIU Member - It’s that time again! CIU is now contacting our Members to request confirmation of your renewal of memberships for CIU’s 2012 on-line Membership Directory. With over 850 CIU Underwriting professionals, CIU is the largest such membership association in Canada. Membership benefits are many and so appropriate in these times of challenges and change. We look forward to your continuation of membership with us. CIU welcomes your membership renewal confirmation. Please review your listing for accuracy, advise us of any necessary changes, and return that membership information, together with your payment, by deadline of January 30th 2012. For members within large firms, please be advised that your department administrators will be contacted regarding CIU memberships on your behalf. Members will be pleased to know that CIU will be holding the line on Membership Fees for 2012 - annual association membership in CIU will remain at $110. 00 CDN, per member, with US Funds accepted at par. Corporate or personal cheque payments only are accepted, no credit cards. Please make cheques payable to - Canadian Institute of Underwriters, referencing CIU Membership 2012. One cheque for large firms is appreciated. Member firms requiring purchase order information, or facilitation of bank draft or direct deposit payments, please contact our administrator: marian@dkci.ca, marian.kingsmill@sympatico.ca If you know of anyone in your organization, or of other industry colleagues who would benefit from CIU Membership, please share the CIU website’s membership link info, directing them to the 2012 on-line CIU Membership application form: http://www.ciu.ca/membershipapplication.html Remember - through the year, it is your responsibility to advise CIU of any change to your membership contact information.
